PRP for plantar fasciitis: what does the current evidence show?

A balanced review of the procedure, comparison treatments, possible benefits and the limitations of current research.

Sharp pain under the heel, especially during the first steps in the morning or after prolonged sitting, is a typical symptom of plantar fasciitis. Persistent pain can substantially affect everyday life, work and sporting activity.

Important context

PRP is discussed mainly for chronic or difficult-to-treat cases. The available evidence does not support a general promise of healing or a guarantee of success.

What is plantar fasciitis?

The plantar fascia is a strong band of connective tissue on the sole of the foot. It runs from the heel bone towards the toes and helps stabilise the longitudinal arch while transmitting forces during walking and running.

The term “plantar fasciitis” suggests inflammation. In persistent cases, however, degenerative changes and repeated mechanical overload are often more prominent than a purely inflammatory process. The term plantar fasciopathy is therefore increasingly used for chronic presentations.

Typical first-step pain

The first steps after getting up or following a longer period of rest may be particularly painful.

Load-related pattern

Symptoms may initially ease after brief walking and then increase again with prolonged loading.

Which factors may contribute to the symptoms?

Plantar fasciopathy usually cannot be traced to a single cause. Several mechanical and individual factors often interact.

  • repeated running, jumping or standing loads
  • a rapid increase in training volume
  • restricted ankle mobility
  • shortened or very tight calf muscles
  • higher body weight
  • prolonged standing or walking at work
  • unsuitable or heavily worn footwear
  • individual foot shape and load distribution

Not everyone with flat feet, high arches or higher body weight develops plantar fasciopathy. These characteristics should not automatically be treated as the sole cause.

How is plantar fasciitis diagnosed?

The diagnosis is usually based on the medical history and a clinical examination. Pain location, load pattern, mobility, foot position and tenderness are assessed.

Imaging such as ultrasound or magnetic resonance imaging may be appropriate when symptoms are unusual, persist despite treatment or another cause needs to be excluded.

Possible differential diagnoses

Nerve irritation, tarsal tunnel syndrome, a bony stress injury or symptoms at the Achilles tendon insertion.

Interpreting a heel spur

A visible heel spur is not automatically the cause of pain and may also occur in people without symptoms.

Which treatments are considered first?

Plantar fasciopathy is primarily managed conservatively. Initial care usually includes education, temporary load adjustment and targeted exercises.

  1. specific stretching of the plantar fascia
  2. stretching of the calf muscles
  3. strengthening exercises for the foot and ankle
  4. manual therapy where relevant mobility restrictions are present
  5. taping for short-term support
  6. individually adapted load management

Insoles may be used as part of a combined treatment plan. As a stand-alone measure they are not sufficient for every patient. Night splints may sometimes be considered where first-step pain in the morning is pronounced.

Additional procedures are generally considered only when consistently performed core measures have not produced sufficient improvement.

What is a PRP treatment?

PRP stands for platelet-rich plasma. It is prepared from the blood of the person being treated.

After blood collection, the blood components are separated by centrifugation. This produces a plasma fraction that may contain a higher platelet concentration than the original blood. A physician then injects the prepared plasma into or around the affected area of the plantar fascia.

Platelets contain various growth factors and signalling proteins. On this biological basis, researchers are investigating whether PRP can influence local repair and remodelling processes.

The biological rationale does not mean that a PRP injection reliably regenerates damaged tissue or completely cures the condition.

PRP is not a uniformly composed preparation

The term PRP covers preparations produced by different methods. They may differ in:

  • platelet concentration
  • leukocyte content
  • remaining erythrocytes
  • the anticoagulant used
  • number and duration of centrifugation steps
  • possible activation
  • injected volume
  • injection technique and accompanying treatment

This lack of standardisation makes comparisons difficult. Results from one protocol therefore cannot be transferred without qualification to every other PRP procedure.

What do current studies show about PRP for plantar fasciitis?

Systematic review published in 2024

A systematic review included 21 randomised studies with a total of 1,356 participants. In the pooled analysis, PRP showed advantages on pain scales compared with corticosteroid injections, shock-wave therapy and placebo.

However, there was no consistent advantage over all comparison treatments for functional outcomes or ultrasound-measured plantar-fascia thickness.

Pain

Several analyses report medium-term advantages, particularly compared with corticosteroid injections.

Function

Some analyses show better functional scores, but results are not consistent across all studies.

Ultrasound findings

No consistent advantage over all comparison treatments was found for plantar-fascia thickness.

Transferability

Different PRP protocols, injection techniques and outcome measures limit clear conclusions.

Comparative meta-analysis

A meta-analysis of 24 randomised studies involving 1,653 participants compared PRP directly with corticosteroid injections. Pain scores statistically favoured PRP after three and six months. There was no significant difference after one month or after twelve months.

Why the results still require caution

Other analyses show that conclusions can vary depending on the statistical model and study design. Earlier systematic reviews also identified partly low study quality, risk of bias and substantial variation in PRP preparation.

Is PRP more effective than steroid injections?

The available evidence does not support a general superiority at every point in time.

Short termCorticosteroid injections may reduce symptoms; after one month there is often no clear advantage for PRP.
Medium termSeveral meta-analyses report better pain or functional scores for PRP after three to six months.
Long termAt twelve months, a clear difference is not demonstrated in every analysis.

This does not mean that PRP is automatically the better treatment for every patient. The decision depends on the diagnosis, duration of symptoms, previous treatment, accompanying conditions, occupational and sporting loads and the protocol used.

When may PRP treatment be considered?

PRP is discussed mainly for persistent symptoms when consistently performed conservative measures have not produced sufficient improvement.

There is no universally applicable minimum duration after which PRP is always indicated. Whether shock-wave therapy or another procedure should be tried first also requires an individual decision.

The foundation remains load management, specific stretching and strengthening exercises, suitable footwear and, where appropriate, orthopaedic or physiotherapy measures.

Which risks and burdens are possible?

Because PRP is prepared from the patient’s own blood, the risk of an allergic reaction to the plasma itself is generally lower than with foreign substances. The treatment is nevertheless not risk-free.

  • pain during blood collection
  • pain during or after the injection
  • a temporary increase in heel symptoms
  • bruising or local swelling
  • bleeding
  • infection
  • irritation or injury to neighbouring structures

Certain medicines, coagulation disorders, infections and other conditions may also be relevant to treatment planning. An individual medical assessment is required.

How quickly might an effect occur?

A fixed onset of effect cannot be promised responsibly. Studies usually assess changes over several weeks and months.

Compared with corticosteroid injections, PRP often shows no clear advantage after one month. In some analyses, differences were not observed until three to six months.

A reduction in pain does not automatically prove that the plantar fascia has completely regenerated structurally.

Technical practice context: PRP preparation

The clinical evidence for plantar fasciitis must be separated from the technical selection of individual preparation systems. Tubes and centrifuges do not determine whether treatment is medically indicated or effective. They are components of a professional preparation process that must follow the intended purpose, instructions for use and internal procedures of the medical facility.

How should PRP for plantar fasciitis be assessed?

PRP is an investigated treatment option for chronic or difficult-to-treat plantar fasciopathy. Several systematic reviews indicate possible benefits for pain and function, particularly in the medium-term comparison with corticosteroid injections.

The evidence remains inconsistent. Differences in PRP composition, injection technique, accompanying treatment and outcome measurement make clear recommendations difficult.

PRP should not be regarded as a guaranteed cure or as the generally best or safest treatment. A sound decision requires a confirmed diagnosis and an individual medical risk-benefit assessment.

Frequently asked questions

Is PRP the first treatment for plantar fasciitis?

Usually not. Initial care generally includes education, load adjustment, specific stretching and strengthening exercises and other conservative measures. PRP is considered more often for persistent or treatment-resistant symptoms.

Is one PRP injection sufficient?

Studies use different protocols involving one or several applications. No number that applies to every patient can be derived from them.

Does the injection have to be ultrasound-guided?

Some studies and clinical protocols use ultrasound-guided PRP administration. Whether this is required in an individual case depends on the physician’s assessment and the procedure used.

Can PRP remove a heel spur?

No. PRP does not remove a bony projection. A visible heel spur is also not necessarily the cause of the symptoms.

Will health insurance cover the cost?

Coverage depends on the insurance plan, medical justification and applicable contract terms. It should be clarified directly with the insurer and the treating practice before treatment.
