PRP cost-benefit analysis: calculate treatments with realistic practice numbers

PRP is used in many practices in aesthetic medicine, dermatology, orthopaedics and regenerative service concepts. Beyond indication, informed consent and professional handling, one practical question matters: how can a PRP treatment be calculated so that material, time, documentation and investment costs are reflected realistically?

Calculating contribution margin: a simple formula

For a first assessment, a clear contribution-margin calculation is often enough. It shows what remains per session after directly attributable costs have been deducted. This can later indicate how quickly a centrifuge or a new PRP setup may amortise.

The word “net” matters. Depending on medical or aesthetic indication, tax treatment may differ. This should be checked with tax advice or a billing partner before prices or treatment packages are published.

Example: if the initial investment for centrifuge and starter equipment is €1,500 and the calculated contribution margin is €195 per session, the mathematical break-even would be reached after about 8 sessions. This is not a profit promise, only a simple business model.

Setting prices: combine market, billing rules and internal costs

Economic pricing should not be based only on competitors. A better approach combines internal cost calculation, physician time, intended positioning, local market environment and correct billing.

  • Describe the service clearly: information, blood draw, preparation, application and follow-up.
  • Separate material costs, medical service and organisational effort.
  • For medical services in Germany, GOÄ or a suitable analogue valuation should be checked.
  • Material expenses should not be charged arbitrarily; GOÄ rules for expenses may be relevant.
  • For aesthetic services without medical indication, VAT treatment should be clarified in advance.
  • Packages should not be worded as a medical success guarantee.

Instead of “3 sessions for a guaranteed result”, a safer wording is: “The treatment scheme is determined according to indication, findings and medical assessment.”

System choice: efficiency comes from standardisation

A PRP system should not be selected only by tube price. Process reliability, handling, required centrifugation time, final PRP volume, documentation, storage and intended purpose of the medical devices used are also relevant in practice.

Systems with separation gel can support the separation of blood components within a defined protocol. The tube, centrifuge and practice protocol must fit together. Economically, a slightly higher material price may still make sense if the process is reproducible, easy to document and reliable in daily practice.
