PRP treatment: procedure, mechanism, benefits and limits
Platelet-rich plasma (PRP) is obtained from the patient’s own blood and is used or investigated in aesthetic medicine, hair treatment and orthopaedics. A factual overview of mechanism, procedure and evidence — without promises of success.

What is PRP treatment?
In PRP treatment, blood is taken from the patient and then processed in a centrifuge. Centrifugation separates the blood components. The aim is to obtain a plasma fraction with an increased concentration of platelets.
The basic workflow is usually similar: blood collection, centrifugation, separation of blood components, extraction of platelet-rich plasma and medical application to the target tissue.
The exact composition of PRP can vary substantially. Relevant factors include blood volume, tube or system type, anticoagulant, centrifugal force, spin time, platelet concentration and the proportion of leukocytes and erythrocytes. These differences are one reason why study results are not always directly comparable.
Mechanism
How does platelet-rich plasma work?
Platelets contain several growth factors and signalling molecules. These are involved in biological processes relevant to cell communication, new blood vessel formation, collagen formation, wound healing and tissue remodelling.

PRP contains several of these bioactive components. At the same time, the literature stresses that effects can differ depending on composition and application. [1]
Important: biological plausibility is not the same as clinical proof of efficacy. The fact that PRP contains growth factors does not mean that every PRP treatment works reliably for every indication.
Composition
Leukocyte-rich and leukocyte-poor PRP
PRP is not a single uniform product. In practice, a distinction is often made between leukocyte-poor and leukocyte-rich PRP.
Leukocyte-poor
Contains only few white blood cells. This variant is often discussed when a lower inflammatory response is desired.
Leukocyte-rich
Contains more white blood cells. Depending on the target tissue, this composition may have different biological effects.
One variant is not automatically better than the other. The relevant factors are indication, target tissue, treatment goal and medical protocol.
Indications
Applications of PRP treatment
PRP treatment is used or scientifically investigated in several medical and aesthetic fields. The evidence base is not equally strong for every application. Effects can differ from patient to patient; a visible or noticeable response cannot be expected in every case.
Aesthetics and skin quality
In aesthetic medicine, PRP is used, among other things, to support skin quality. Common topics include fine lines, declining skin texture, scars or combination with procedures such as microneedling.
In aesthetics, PRP is sometimes colloquially called a “vampire lift”. The more precise medical terms are PRP treatment or autologous treatment with platelet-rich plasma.
Studies and clinical experience suggest possible positive effects in selected aesthetic applications. However, results differ individually and are not guaranteed.
Before-and-after images should not be used or linked for this application, especially in aesthetic procedures without medical necessity. [7] [8]
PRP treatment for hair loss
A frequently studied field is PRP treatment for alopecia, especially androgenetic alopecia, meaning hereditary hair loss.
PRP is injected into the scalp to influence biological processes around the hair follicles. A systematic review and meta-analysis published in 2025 evaluated 43 randomized controlled trials with 1,877 participants. The results suggest a possible benefit in selected forms of alopecia, including hair density and patient satisfaction. [3]
Important questions remain open: optimal preparation, activation, number of sessions, intervals, long-term effects and patient selection. PRP may support selected forms of hair loss. There is no guarantee of new hair growth.
Orthopaedics and sports medicine
In orthopaedics, PRP is mainly investigated for joint complaints, tendon irritation and sports medicine questions. Knee osteoarthritis, medically called gonarthrosis, is discussed particularly often.
Several studies and meta-analyses show possible positive effects on pain and function, especially in mild to moderate knee osteoarthritis. Results are not fully consistent because PRP products, protocols, patient groups and study methods vary considerably. [4] [5]
A blanket statement such as “PRP cures osteoarthritis” would be wrong. PRP cannot reverse osteoarthritis. It is being investigated as a possible option for symptom control and functional improvement.
Other potential uses
PRP is also being studied in other areas such as dentistry, wound healing and reconstructive medicine. For some uses there are initial studies or clinical experience; for others the evidence is still limited. General promises of efficacy outside established indications are not appropriate.

Procedure of PRP treatment
The exact procedure depends on the indication, the system used and the medical protocol. A PRP treatment usually follows several steps.
Medical assessment and informed consent
Before treatment, it should be checked whether PRP is suitable for the specific situation. This includes medical history, diagnosis, possible contraindications, medication use and realistic expectations.
Blood collection
Blood is drawn from the patient. The required volume depends on the PRP system and the treatment area.
Centrifugation
The blood is processed in a centrifuge. Blood components are separated by centrifugal force. Differential centrifugation is a core principle of PRP preparation. [2]
Extraction of PRP
The platelet-rich plasma fraction is collected. Depending on the system, the resulting PRP may differ in purity, cell composition and platelet concentration.
Medical application
Depending on the indication, PRP is then injected or otherwise applied medically. In orthopaedics, imaging guidance such as ultrasound may be useful.
Assessment
Possible benefits, limits and risks
PRP treatment is based on autologous starting material. This may offer certain potential advantages. These points should not be understood as promises of success.

A 2025 meta-analysis on PRP injections for knee osteoarthritis describes PRP complications as similar to corticosteroid and hyaluronic acid injections, but more frequent than placebo injections. [6]
Medical caution
When should PRP treatment be avoided or assessed carefully?
Whether PRP is suitable must be decided by a physician.
Particular caution in
acute infections, certain coagulation disorders, relevant systemic diseases, cancer or a relevant history, pregnancy or breastfeeding, certain medications, unclear diagnosis or unrealistic expectations.
Patients should not stop medication on their own. Any medication changes belong in medical hands.

The costs of PRP treatment can vary widely. Relevant factors include:

A general price estimate is therefore not very useful. Patients should receive a transparent cost overview before treatment and clarify whether the costs are privately payable.

Many patients search online for “PRP treatment experiences”. Such reports can provide an initial impression, but they do not replace medical evidence.
Individual positive or negative reports cannot automatically be transferred to other patients. Diagnosis, indication, protocol, quality of PRP preparation, medical experience and realistic counselling are more important.

Frequently asked questions about PRP treatment
Is PRP treatment painful? +
The treatment can be uncomfortable, especially in sensitive injection areas. Symptoms are often temporary. Depending on the application, the physician may use measures such as cooling or local anaesthesia.
How quickly can results be seen or felt? +
This depends on the indication. In skin or hair treatments, possible changes may only become visible after weeks. Orthopaedic effects are also usually not assessed immediately. Not every patient responds to PRP.
How many sessions are required? +
There is no general answer. Some protocols use one session, others several sessions at intervals of weeks. The decision should be made medically and according to the indication.
Is PRP natural? +
PRP is obtained from the patient’s own blood. “Natural” does not automatically mean risk-free or reliably effective. Autologous procedures can also have side effects and must be performed professionally.
Is there a guarantee of success? +
No. Serious medical counselling should not give a guarantee of success. Studies show possible positive effects depending on indication, but not reliable efficacy in all patients.
