Treatment of cellulite with PRP (platelet rich plasma)

What is cellulite?

Cellulite is one of the most common imperfections in women of childbearing age.

When fatty tissue accumulates excessively in the subcutaneous tissue and retains water, the so-called cellulite occurs. In fact, its formation mechanism is related to microcirculation dysfunction, that is, low blood flow in these areas, which tends to form a fibrous membrane that traps fatty tissue and water.

Cellulite classification

Not all cellulite is the same, it can be divided into three stages according to severity and cosmetic effect:

Stage 1 - Edematous cellulite

The differences between water retention and cellulite

Many people confuse cellulite with water retention. The definition of water retention is the excessive accumulation of fluid between cells that is unevenly distributed and accumulates between tissues.

On the other hand, cellulite is the result of vascular inflammation that affects fatty tissue and interstitial tissue. Therefore, if you suffer from water retention, it is not necessarily the result of cellulite. 

The same is true for the opposite: the presence of cellulite is not considered a sign of a problem with the lymphatic system.

When should you resort to treatments?

One of the classics is mesotherapy, which uses lipolysis and dehydration medications, and there are more innovative therapies such as carboxyl therapy or PRP therapy, which aim to restore proper local microcirculation, eliminate excess fluid stagnation and improve tissue fibrosis.

PRP treatment for cellulite

PRP treatment is the most powerful way to treat cellulite and ensure that your skin regains a smooth surface.

Since there are multiple causes of cellulite, PRP is used as a complementary treatment while providing the best possible results. 

Once properly diagnosed, PRP is obtained by drawing your own blood with special PRP tubes and processing it to extract a high concentration of growth factors (these are important for treating the cellulite areas). 

The process of reinjecting your own blood plasma into the skin is based on the process of biostimulation, which is the activation of the skin's biological functions with the aim of smoothing its texture, resulting in aesthetic improvement.

PRP has the ability to produce growth factors that signal collagen molecules to revitalize the skin area, making it smoother, firmer and tighter. When treating cellulite areas of the skin, PRP therapy helps to smooth out the hollows in the skin and also contributes to firmness. 

Implementation of PRP treatment for cellulite

A peripheral venous catheter with a diameter of at least 1.1 mm (the diameter depends on the injection site) is used to collect blood in a volume of 18-36 ml in 2-4 PRP tubes. 

During centrifugation, blood is divided into two main parts: red blood cell-leukocyte clot and platelet-containing plasma. Use a syringe to remove the platelet supernatant, which is located in the upper part of the tube above the separating gel. 

There are different stages of cellulite. To correct soft and edematous cellulite (stages I and II), the use of autologous platelet plasma is effective. Autologous plasma stimulates microcirculation and lymphatic drainage, has a nourishing and firming effect.

Plasma injection is painless and after the operation there is a slight itching, which disappears after 3 hours without special treatment.

The main mechanism of edematous cellulite (stage I) is related to vascular insufficiency and interstitial fluid retention. To correct stage I cellulite, 36 ml of blood (approximately 4 PRP tubes) is taken from the patient, from which an average of 16 ± 1 ml of autologous platelet plasma is obtained. This volume is sufficient to inject the thighs and buttocks.

Autologous platelet plasma is injected into the posterior, anterior, inner, and outer thighs, with an injection distance of 2.5-3.0 cm.

More frequent injections are not necessary, as the effective area of autologous plasma derived from platelets is 3-4 cm. The infiltration technique of stage I cellulite is optional.  With it, autologous platelet plasma is injected into the cellulite area.

Due to the significant improvement of microcirculation and metabolism, the appearance of cellulite will decrease and the volume of the thighs and buttocks will decrease (due to the removal of intercellular fluid).

Due to the effect of platelet growth factor, it can increase skin elasticity, reduce sagging, and reduce chronic inflammatory symptoms characteristic of pudendal wrinkles. 

Monotherapy is effective in the first stage of cellulite, but for localized fat deposits, such as the "nose" of the thighs, it is recommended to use a lipolytic cocktail in combination. In this case, the treatment plan consists of three stages with an interval of 7 days between stages:

  1st stage - inject autologous platelet-derived plasma into the correction site.

  2nd stage - introduce lipolysis cocktail into the correction zone 6-8 times a week

  3rd stage - double injection of platelet derived autologous plasma once a week.

With this combination of procedures, it can not only reduce the fat in the problem area, but also improve the volume and elasticity of the skin.

PRP treatment for light cellulite

Light cellulite (stage II) is characterized by the general loss of tension in the skin (including subcutaneous tissue) and lower muscles, at this stage fat cells converge and surrounding collagen fibers become denser. To correct cellulite at this stage, it is recommended to combine PRP therapy with the introduction of lipolytic mesoderm therapy.

PRP treatment for hypertrophic scars
