What is a facelift with stitches?

A thread lift is an effective, non-surgical technique for facial rejuvenation. During the treatment, absorbable and non-absorbable revitalizing threads are inserted into the skin through a puncture using a special atraumatic cannula.

Composition and mechanism of action of the facial thread

The non-absorbable sutures are made of polypropylene, a synthetic suture material that does not cause allergic reactions. The absorbable suture material consists of E-Caprolac (a suture material that can be completely absorbed by the body) and L-lactide. Polylactic acid promotes the production of collagen and has additional rejuvenating effects at the cellular level.

The external effect of polylactic acid is delayed, i.e. it becomes visible only about 2-4 months after the procedure and reaches its peak after about six months. During this time, new endogenous collagen fibers are formed in response to the presence of the injected thread (neocollagenesis). This process of natural tissue formation continues despite the complete resorption of the thread's microparticles. The effect of thread lifting lasts for at least 2 years.

Duration of resorption of the suture material

Depending on the manufacturer, the threads have a different resorption time. On average, it takes between 8 and 14 months for the threads to dissolve. During this time, fibrosis and collagen can form, which makes the construction stiffer. The skin will then be less ptosis in the future. This effect lasts for a long time.

Rehabilitation period

On average, the rehabilitation phase lasts from 7 to 14 days, depending on the individual characteristics of the body and the level of stress.

The thread integrates for up to 40 days. During this time you should avoid strong skin movements and pronounced facial expressions.

" Swelling can be divided into acute and subacute. Acute swellings subside within about 7-8 days, subacute swellings follow within 2-3 months, so the final result is much later.

" Hematomas and swellings. This may occur after surgery, but will go away on its own over time.

" Pain may occur at the tip of the thread, at the point of fixation of the thread. In the course of healing, all unpleasant sensations disappear.

" Bumps at the point where the suture is inserted When the suture is inserted, the layer around it swells. It may not build up evenly and may become somewhat lumpy. This does not mean that the suture was not inserted correctly. You will need to wait for the swelling to subside (2-3 weeks on average), the function of the skin and the alignment of the result to normalize

" Restrictions. Within a week and a half it is recommended not to visit saunas, baths and solariums. Heat treatments in the places where the threads were implanted are prohibited for one month, massages, physiotherapy treatments and anything that disturbs the integrity of the skin (mesotherapy, injections, etc.) for two months.

Surgical lift or thread lift?

A facelift leads to a more pronounced result immediately after the procedure. And it is because of the expression that the effect is lasting. When the threads are used, they have a different mechanical effect: they change the structure of the skin. And the effect is less pronounced, but very permanent. Often seeing patients who had stitches placed 7-8 years ago, the effect is often more permanent than after a complete facelift. That is why it is recommended to many patients a combination of facelift and stitches.

It is all about the mechanics. When a classic facelift is performed, the tension is on the SMAS layer (the layer under the skin, under the subcutaneous fat), and it can be moved to a distance of 1 to 3 cm. The skin is already connected to the SMAS layer, so there is tension. When you apply the sutures, the skin itself is stretched and the subcutaneous fat layer is slightly displaced. Any suture, whether absorbable or non-absorbable, will cause fibrosis. Fibrosis is a tissue reaction that results in the formation of a vector-thin scar (or collagen) along the suture. This collagen is essentially scar tissue that forms a stiffer skin structure. Consequently, the patient is less prone to ptosis.

FAQ about thread lifting

Can sutures produce a pronounced and permanent facelift effect?
There is a misconception that the sutures will provide a permanent lift if inserted by the "right" doctor. In fact, during the first few weeks after insertion, the threads cause only minimal tightening as long as the incision holds and swelling is present. And the thicker the subcutaneous layer and the better the skin tone, the faster the tightening effect disappears. The main function of the sutures is to hold the skin in place. In other words: If you start threading at the age of 40, your face will be less puffy at 45 than if you had not threaded. So threads are very good and right, but don't expect them to give you a lift, they only significantly slow down the appearance of signs of aging. The exception is if you insert threads with a twist, a loop, there can be a lifting effect for a longer period of time (a few months).
Does my skin need any special preparation for a thread lift?
No special preparation is required. Patients should especially carefully consider their schedule after suture cutting, as it is impossible to guarantee that there will be no bruising, bruising or swelling.
When should I repeat the thread lifting procedure?
Usually patients like the effect and after about 2 years they come back for a "refresher". After 2 years or so, it really is possible if there is a reason for it. But if the effect is still there and the skin is smooth and everything looks good, then it is not necessary to stress the patient further and the re-filament lift can be postponed.
Can a facelift thread break?
Of course, if you take the thread in your hand and pull on it, it can break. But your facial muscles can't exert so much tension that the floss will break, no matter how hard you try. So there is no reason to be afraid of it. There may be another issue: the thread itself has fixation "claws" and this is where some of the fixation may come loose. Not too much, but it is possible. To minimize this effect, the long threads are used. Because the longer the thread, the more centimeters and these "claws" can distribute the load and then the effect will be more uniform
Won't the sutures interfere with future surgical lifts?

Can the threads (notches) shift?
The threads always shift slightly if the doctor does not reverse the thread direction. For this reason, the thread is slightly overtightened.
What should I do if the tip of the thread is contoured?
Wait until the swelling goes down a little, and then go to the doctor who pulled the stitches. The procedure for removing the tail of the stitches is simple: pick up the tail, pull it out as far as possible, push it into the skin and cut it off with scissors. The tail is then displaced about 5 mm or even a centimeter deep, and that solves everything.

Applications for different types of threads

A thread lift is one of the most effective and efficient methods of cosmetology, which can permanently preserve and prolong the youthfulness of the skin.

There are three main types of sutures: polydioxanone (PDO), polylactic acid (PLA) and polycaprolactone (PCA). PDO sutures are made from a synthetic, biodegradable polymer that has been used in surgery for many years. When sutures are placed under the skin, they can compress and lift sagging or droopy areas in various parts of the face and body to reduce the effects of gravity and aging.

Understanding this procedure can be complicated because there are several types of threads for different applications.

Mono threads

Mono threads are made from PDO. They are smooth threads without serrations that are placed on the face in a mesh pattern to tighten the skin and improve and stimulate collagen formation around the threads. Skin tone improves as collagen begins to form around the threads. This type of thread is ideal for treating sagging skin on the neck, forehead wrinkles, sunken cheeks and expression lines.

To withstand a lift, the threads are attached to the "anchor point" of the scalp or face. Mono threads tighten the skin, but without lifting it significantly. Therefore, they should be used in conjunction with cog strings to achieve an enhanced facelift and rejuvenation.

Typically, 10 to 50 monofilaments are needed for one correction.

The smallest gauges are used to correct superficial wrinkles and only in cases where the skin only needs to be tightened and not transported. The threads with the larger caliber are used for work on the neck to correct sunken cheeks.

Fields of application:

- Neck wrinkles, elimination of neck sagging;

- Forehead, cheekbones, correction of sunken cheeks;

- drooping corners of the lips;

- Under eye area, elimination of expression lines;

- Correction of drooping eyelids;

- Asymmetry of the face;

- Displacement of the oval line.

Mono threads are usually inserted with thin, sharp needles. Mesonite calibers: 30G, 29G, 27G, 26G, 25G.

The right choice of instruments allows you to make adjustments in sensitive areas without the risk of causing complications or leaving bruises.

Serrated threads - Cog threads

Cog yarns have prongs that "cling" to the skin to support and lift the fabric. Cogs, unlike monofilaments, do not require an attachment point. The process of collagen formation occurs around the threads and their prongs.

Applications: most effective for chin lift, forehead lift and nose lift.

The Cogi thread material comes in:

- RLA - these extra strong threads contain polylactic acid. It is this acid that promotes collagen production;

- PDO - these threads are made of polydioxanone, which ideally adapts to the tissue;

Also to be distinguished:

- Double cog - notched on both sides, so it keeps its shape perfectly even with a lot of soft tissue;

- Uni one side notched - Cog - with differently oriented notches present only on one side. Relevant for younger types of leather;

- Bi inc isions - in this case, the incisions on the thread are arranged in a circle. Effective in solving the problem of reticular folds.

The technique is especially suitable for patients between 30 and 45 years of age.

The lower and middle part of the face can be corrected with 19G and 23G sutures. The choice of caliber depends on how deep the wrinkles are and how large the area of skin to be moved is: while a 19G caliber can be used to lift the eyebrows, a larger caliber is needed to correct sagging cheeks and shape cheekbones.

Screw thread - Screw

Screws can have the form of one or two threads interwoven around the introducer needle. The screw thread has a great effect on creating a voluminous effect in the area of the facial oval. The production of collagen around the threads and their prongs helps restore volume and improves the texture and elasticity of the skin, resulting in a natural aesthetic result.

Applications: sriu threads are commonly used for a general facelift and to add volume to the nasolabial folds, lip chin wrinkles and oval face contouring. sriu threads can also be used to correct the breast, neck and abdominal areas.

They are used for subdermal injection into areas with moderate wrinkles and folds. The spiral arrangement allows for secure fixation in the tissue. Screw threads are mostly used for the sizes: 30G, 27G, 26G, 29G.

The largest gauges are used for body lifting. The age of the patient should be considered. Women younger than 35 years usually do not need such sutures.

Spiral yarns - Twin Screw

Twin Screw thread is a new generation of mesonite threads. They have a pigtail structure that spirals around a laser-cut needle. The threads help to achieve a pronounced effect on all skin types. They are especially suitable for people who have a very pronounced fat severity. It is optimal for thick, fat people.

Applications of the Twin Screw:

- Correction and elimination of a double chin,

- Sagging of the cheeks and nasolabial folds,

- For the correction of the lower face to give it a soft oval shape.

The following thread gauges are available on the market: 25G, 26G, 27G. The threads of small calibers allow to perform corrections in zones: Correct nasolabial folds, get rid of "frills". At the same time, the larger gauge is suitable for correction of cheeks and facelift. Long threads are used for tightening hips, breasts, abdomen and buttocks.

Double threads on needle shafts - Dual arm

The double thread (mesonite on two needles) is an innovation in body frame lifting. These threads provide a long-lasting lifting effect by displacing the tissue in combination with a firm anchoring point, which tightens the skin and strengthens the ligaments.

The applications of these threads are circular facelift, neck lift. By choosing denser and longer threads, it is possible to use them for breast and abdominal lifts at the same time.

The basic calibers used in cosmetology are: 25G, 26G, 27G. They belong to the second generation of mesonites. They are interwoven, which gives them maximum strength. Due to their particularly large diameter, they may only be used in areas with heavy and coarse skin that require highly effective lifting.

The double arm threads dissolve within a year as they are replaced by connective tissue fibers, which extends the lifting effect up to 2 years;

Application areas of double arm threads:

- Lower face, under the chin area;

- Smoothing of nasolabial folds;

- Forehead lifting;

- Shaping of the facial contour.

The double-arm thread is suitable for shaping the facial architecture. It is an excellent option for patients with severely deformed contours.

The thread is available in sizes 19, 20 and 21G. When a specific thread is selected, the corresponding lifting pattern is selected in parallel. The smallest gauge is suitable for smoothing wrinkles and correcting slight creases. Caliber 21G can be used for body work.

Multi-thread (multi-thread)

Bundles are powerful collagen stimulating sutures consisting of multiple sutures in a single cannula. They penetrate multiple areas of the subcutis and therefore significantly increase the amount of collagen compared to a single thread.

The introduction of thread connections triggers regeneration and restructuring processes that are many times more powerful and effective. The effect of filaments is much more pronounced than that of monofilaments. Filaments can be applied to wide and flat surfaces to give them more elasticity.

Areas of application:

- Forehead, cheeks;

- Wrinkles under the eyes;

- Neck wrinkles;

- Double chin.

Bundles of seven double folded threads are inserted into the subcutis of the nasolabial fold or the lip and chin wrinkles. The so-called "bulldog cheeks" are also corrected with these sutures, and the contour of the lower jaw is changed if necessary. A size 23G (60 mm) needle is used.

Filaments are considered the heavy artillery in the world of mesonite. They are used not only to correct deep wrinkles and eliminate a double chin. Longer filaments are actively used for body tightening and contouring. They can be used for the buttocks, breasts, abdomen and for a more pronounced waist.

Thread sizes

When choosing the length and type of thread, beauticians are guided by the thickness and type of skin. The most common lengths are 38 and 50 mm. Depending on the desired effect, linear (smooth), spiral, serrated (notched) are used.

For the correction of the oval of the face and the décolleté line, it is better to use a simple linear thread of 25-90 mm. Spiral-shaped ones are used to eliminate a double chin, lift eyebrows, smooth deep wrinkles and lift facial contours. Serrated threads are used for pronounced ptosis and deep wrinkles and have a lifting effect comparable to a surgical lift.

The size of the thread is determined as follows:

- Thickness of the support - volume range from 25G to 30G;

- Length of the thread - range from 25 to 150 mm.

Long threads perform body lifting. Thin needle and short thread are intended for work in the periorbital area.

The size also depends on the modification: Twin is a classic medium volume material, while Screw is used to tighten the skin or build volume.

It is recommended to use it for different areas of the face and neck:

- In the periorbital area, it is advisable to use 30G miniature needles and 25mm or 38mm smooth threads, up to 20 needles;

- For the elevation of the zygomatic bone, bidirectional 19G (100mm) sutures. One thread is placed 3cm above, the other 3cm below the cheekbone angle. For the stability of the result - supplemented with 30G (38mm) and 30G (25mm) linear threads - (15-20 pieces);

- for smoothing the forehead - linear or spiral mesonite 38mm-50mm on needles 27G and 29G (15-20 pcs.)

- Neck and décolleté - placement of 30-40 threads along the lower line of the oval, recommended sizes - 27G (50mm), 26G (60mm);

- to eliminate ptosis of the middle and lower third of the face - the spiral mesonites are placed in rows in a crossed arrangement. Sizes - 27G (50mm) or 27G (60mm) - (20-30 pcs.);

- Body areas - smooth threads with length and needle volume of 27G (60mm), 26G (90mm) - (40-100 pcs.)

- Along deep folds the installation of 26G (60mm) spiral threads is indicated.
